Initial Comments:
Based on the findings of an offsite followup survey completed December 18, 2023, Integrity Home Care, LLC was found to have corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiency was cited as a result of a complaint investigation completed October 29, 2023.
